One of the most common concerns for people with hypothyroidism is maintaining proper body weight. For people with Hashimoto’s (the most common cause of hypothyroidism) this comes in 2 varieties. They gain weight and can’t lose it or they have trouble keeping it on. In this post we will examine the many reasons why a lot of people with hypothyroidism have such difficulty losing weight. The Thyroid Influences Metabolism. One of the obvious things that people think about with hypothyroidism and weight gain is the fact that the thyroid has an impact on the body’s metabolic rate. What does this actually mean? In technical terms, metabolism is the amount of oxygen used by the body over a particular amount of time. When this measurement is made at rest, it is called the basal metabolic rate or BMR. Testing BMR was, once upon a time, used to assess a patient’s thyroid status. If you had a low BMR, your thyroid was underactive, and if you had a high BMR your thyroid was overactive. Later studies showed that low thyroid hormone levels were linked to low BMRs. Then, most physicians decided to scrap testing BMR in favor of simply testing thyroid hormone levels because it is easier and it was found that the thyroid was not the only thing to influence metabolism. Not So Fast. High or low BMRs are associated with changes in energy balance. Energy balance comes down to the difference between how many calories one eats and how many calories one’s body burns. Things that create a high BMR, like amphetamines, for example, often cause a negative energy balance which results in weight loss. And our ancestors evolved in a calorie poor environment where fat was pretty hard to come by. As a hunter gatherer on the open plains of Africa, our forefathers (and foremothers) had to expend a lot of energy to get food and there wasn’t a whole lot of fat around. Most prey was pretty lean and grass fed and there weren’t too many fast food joints (The fossil record has yet to reveal a single Mickey D’s). As a result, our bodies developed a natural tendency to store whatever fat was available. And that fat got programmed with some pretty ingenious innate intelligence. One of those ingenious adaptations from the clever mind of fat is the hormone leptin. Leptin, The Body’s Fat Programmer. ![]() Leptin is a hormone that is made in your fat cells and it is involved in maintaining body weight. Interestingly enough, it also has influence on the thyroid. Leptin acts as an important control system that communicates to other organs about the state of your fat balance and whether to eat more or stay in low- metabolism survival state. But with Hashimoto’s and hypothyroidism, lots of things are often not working properly and many people develop leptin resistance. Leptin Resistance, Not Unlike Insulin Resistance. You may have heard of insulin resistance (if you haven’t, read about it here: http: //www. Well, leptin resistance is similar and often co- exists. In fact, both are consequences of obesity. With insulin resistance your body’s insulin receptors get fatigued because they have to deal with so much sugar. A lot of people in the US, today, have some degree of this. With hypothyroidism, people can become overweight and they tend to use less energy. The increased amounts of fat and the lower energy use can result in leptin resistance and you wind up with a vicious cycle where leptin stops doing its job. It stops telling you when to eat and it stops signaling your thyroid. Dieting Can Make This All Worse. Of course, many people’s natural inclination when they gain weight is to go on a diet to try and lose it. Often these people will keep dieting and fail and then diet again and get all stressed out about it because it’s not working. And guess what? Chronic dieting and/or major stress are common causes of leptin resistance. As a result, leptin no longer signals your hypothalamus and your metabolism slows down. Leptin resistance makes the hypothalamus believe that you are in starvation mode, and you make more fat, and slow down thyroid hormone production. So, TSH goes down, you don’t convert as much T3 from T4, and your reverse T3 goes up. And, in what can only be described as unfair and cruel, your appetite actually increases, you become insulin resistant, and fat breakdown (lipolysis) slows down. So a vicious cycle is created in which more fat accumulates, you’re hungrier and your thyroid is slower. Over time, you gain weight, especially around the mid- section, and it becomes more difficult to lose the weight and accumulated fat. Leptin Resistance Leads to Inflammation. With Hashimoto’s (and hypothyroidism) one of the most serious problems is inflammation. In fact, a destructive inflammatory process is really what is at the root of all autoimmune diseases (of which Hashimoto’s is one.)Leptin controls and influences the immune system, too. It is chemically very similar in structure to IL- 6, which is an inflammatory cytokine (immune protein) that studies have shown to be significantly elevated in women with Hashimoto’s. One of the places where you can find high concentrations of IL- 6 is in the fat that accumulates around the abdomen. This adipose tissue is highly inflammatory and can, itself, lead to the progression and more aggressive proliferation of many diseases. Low vitamin D has also been associated with both insulin and leptin resistance. And vitamin D is an important anti- inflammatory that is often low in people with hypothyroidism. This Is Your Brain on Leptin. Your perception of hunger is intimately linked to your brain chemistry. Normally, when things are working properly, your hypothalamus gets signals that you need energy and a brain neurotransmitter called neuropeptide Y (NPY- not to be confused with NYPD) is released. It makes you want to eat more carbohydrates (think overwhelming urge to finish that can of Pringles). That surge is what makes you feel cravings and hunger. Once your body has had enough carbohydrates, the brain releases serotonin which is your brain’s way of saying, “Put down the bag and step away from the counter.”Studies have shown that NPY is an important go between of leptin in the central nervous system and the hypothalamus. And that giving people NPY suppressed circulating levels of thyroid hormone (T(3) and T(4)) and resulted in an inappropriately normal or low TSH. So, high levels of NPY can actually lead to functional hypothyroidism. And leptin’s job is to suppress NPY. So once again, we have the makings of another vicious cycle. This also may be yet another reason why TSH testing can be unreliable in circumstances involving leptin and insulin resistance and weight gain due to hypothyroidism. As we saw above, one of the many roles of serotonin is to tell you to stop eating those crazy carbohydrates. Thyroid hormone and serotonin have an intimate relationship and many studies have shown that thyroid hormones impact virtually all neurotransmitters in the brain. So, with hypothyroidism you also may have less serotonin production and all the accompanying emotional and physiological problems related to that, like depression and minimized signaling that tells you to stop eating. The cravings don’t go away, they intensify with weight gain and hypothyroidism. And this is all accompanied by emotional discomfort that makes you want to reach for that high carb comfort food. Help! What Do We Do? Naturally, we can’t leave you there, depressed and jonesing for comfort food. Let’s talk about what to do about all of this. In a simple sense the root of all of this can be summed up with one word: inflammation. Being overweight is a problem of inflammation. So is Hashimoto’s, the most common cause of hypothyroidism. So the most important thing to do is to reduce inflammation. And if you do that you can start to unwind many of these hormonal and neurotransmitter disruptions that are leading you down the road to feeling really crappy a lot of the time. Paleo to the Rescue. One place to start is with some version of the Paleo diet. There are many versions, with my patients I use a version that is tailored for people with autoimmune disease. This is also called the elimination diet and is very restrictive. But it is also very effective. Desperate times call for desperate measures. If you want to unwind all of these vicious cycles and to reset leptin and insulin you can’t mess around and use half measures. When things get bad, half measures do not result in half results. They result in disappointment or, worse, no results. This diet involves the elimination of virtually everything that is inflammatory in your diet and it removes almost all of the carbohydrates that lead to most of the problems we have described, too. This allows your body to convert from a sugar burning leptin and insulin resistant machine to a happy fat burning ecosystem. It also reduces systemic inflammation. This is absolutely essential, but often, the diet is not enough. As we have seen here, many systems are involved (and this is just the tip of the iceberg- click here to learn more: http: //www. Over time, these various systems start to break down because of the influence of thyroid hormone on virtually every aspect of our physiology. The impact of hypothyroidism is felt everywhere. This can cause problems in all the other systems of your body including your adrenals, your liver, your heart, your pancreas, your brain, your blood and much more. Other Important Anti- inflammatories. Hypothyroidism Diet and Training for Weight Loss. Inked by Leigh. It needs to be stated right off the bat that this is a very large topic and couldn’t possibly be covered via one article. The goal of this article is the following: Give you a base understanding of what hypothyroidism is. Give you an understanding of what hypothyroidism isn’t. Give you a base understanding of the current best options for testing. Provide my opinion of the best course of action for fat loss if you have a thyroid problem. What does your thyroid do? Your thyroid is located in your neck and produces hormones that are important to various processes in the body. This ranges from oxygen use, heat production, and use of nutrients and vitamins. That is just touching on a big player to many important processes. What is Hypothyroidism? When T4 (thyroxine) drops too low it causes different processes in the body to slow down and become less efficient. The symptoms of this range based on the severity of the illness. The more severe the decrease in T4 the worse the symptoms. The symptoms for hypothroidism are vague and can apply to many situations. For example, depression and fatigue can be symptoms of many different diseases. I have caught heat in the past for saying that tests show if you have a thyroid problem, and they do. That doesn’t mean I don’t believe that doctors can’t mess it up. If you use this guide the next time you go to the doctor then you should at least get the test information you need. Before you get your tests done talk with your doctor about the information you have and don’t be afraid to go somewhere else if you need to. Ask your doctor to check these levels to cover as much ground as possible. TSHFree triiodothyronine (f. T3)Free levothyroxine (f. T4)Reverse T3. Total T3. Total T4. Antithyroid antibodies. There are other tests such as imaging and urine tests, but in my opinion these tests should at least give a signaling to something being off in your system for further investigation as needed. How to read the results. The problem with the results is that different labs have different ranges. It also isn’t the levels, but how the levels correspond with one another. Currently your best bet for feeling optimal is to have your TSH landing at the lower end of the normal range and your T3 & T4 at the upper end of the normal range. You can have normal TSH levels but low T4 levels and this could mean you are entering into a subclinical phase. Subclinical does not mean full blown. I have seen clients go from subclinical to normal levels from simply taking proper diet breaks. In fact that is one purpose of The Metabolic Repair Manual. Starve Mode. This is not about self diagnosis though, you want a doctor involved. This is merely about looking out for yourself and lowering any frustrations. If you come back with levels you feel or your doctor feels is not optimal, then you need to be ready for the next stage of action. What to do if you are diagnosed with a thyroid problem? The first thing you need to do is be very clear in your communication with your doctor. If this isn’t your doctors field of knowledge you will be much better off getting a referral to an Endocrinologist. Medication may or may not be needed depending on your levels. Below is a brief explanation of what you can do for different stage of diagnosis. Hypothyroidism – If you are diagnosed with full blown hypothyroidism with very extreme and obvious levels then chances are you are going to be on medication for life so that you can function well. Don’t let this scare you though. If you are communicating with a doctor who is knowledgeable this does not have to be a bad event. When treatment for hypothyroidism is done correctly you merely feel better. The problem is when the patient and the doctor aren’t communicating right and dosage levels are wrong. Make sure to explain to your doctor how your medication is making you feel and discuss with them the specific issue and signs for too high or too low of a dosage for your medication. Nonthyroid Illness/Subclinical/Euthyroid Sick Syndrome. All basically mean that you are messed up, but not necessarily broken. Most people who are dealing with less than optimal thyroid function are in this category. Ever since Oprah proclaimed her thyroid woes we have books piling the shelves (and internet ebooks) about how it “isn’t your fault.” Men and women everywhere are running out and getting themselves diagnosed with full blown hypothyrodism, when it may not be the case at all. There are a lot of things that can contribute to TSH levels being normal, but other levels being low. One in particular is constant dieting down and overtraining, but this doesn’t mean you have hypothyrodism. As I stated earlier when I wrote the Metabolic Repair Manual the program had two main goals.- Help people reload on calories with as little side effects as possible.- Help normalize low aggression hormone problems like thyroid and menstrual problems. To give you a cliffs note on the book the goal is to train smart and treat your body well. I merely constructed a step- by- step method of doing that and discuss how constant dieting down can harm your metabolic burn, but that doesn’t mean you are broken yet. That doesn’t mean medication should or should not be used, but I would look into other options first and make sure it is what is needed.“But isn’t my thyroid making me fat!?”Hypothyroidism is blamed often for weight gain in both adults and children. With exception of some water retention in more severe cases weight gain isn’t usually an issue. Energy balance still works in those with thyroid dysfunction, it just may be lowered. If you are treating yourself with proper medication, or if you take the time to take proper breaks and refeeds from dieting, it really shouldn’t be an issue. Only extremely rare diseases or life threatening illnesses can cause massive osmosis and retention, and this is followed by hospital stays or death. The most you can expect with slow thyroid function is either a higher tendency to water retention on low levels or slower metabolic function causing your rate of burn to be slightly lower than normal. Dieting and training for hypothroidism. It is exactly the same as it would be for anyone else except you have to be smarter, listen a little harder to your body, and the chance of having to take things a little slower. In short, you don’t really get lucky and can’t get by on crap habits. With any dieting down situation you want to abide by a balance of enough deficit to achieve results, but smart enough training and diet breaks to keep things going along smoothly. If you are dealing with thyroid issues that there are a few things to keep on the front of you mind. Watch training volume and keep it lower. I would not recommend lots of HIIT mixed with even a moderate deficit (2. Schedule re- feeds a little more frequently. The harder you diet down, the more you need to break. Don’t be overly carb restrictive for extended periods of time. If sensitive to dieting down I recommend keeping to a very minimum protein intake (. Ideally a carb cycling program is best, however due to water retention issues you are going to be more likely to retain water on your “ups” so keep this in mind. You can diet down just as low as anyone else though. If on medication it should be much easier, but make sure to keep up with your doctor if dieting down for an extended period of time. Overall you would follow the same core beliefs that are in Fat Loss Troubleshoot. If you are someone that has been having long term overtraining and dieting down issues and think that your low levels or issues may be because of that then I highly recommend getting Starve Mode. The constant posts about how lectins are the devil and how we need to keep our carbs to less than 5. Good Lord! How can one even eat a small stack of pancakes on 5. One pancake equals about 1. Not to mention the maple syrup. Just eating a couple of innocent pancakes blows your allocation of carbs for the whole day! One sandwich on sprouted whole wheat bread uses up HALF your carbs for the day. Paleo is the hottest trend going right now. I've got no issues with Paleo folks. If it works for you, more power to you! It may seem like an old way of eating since it harkens back to the Stone Age, but the fact is, we don't really know what paleolithic man ate. There's new evidence showing that hunters and gatherers were eating grains 1. Bottom line: Low carb paleo didn't work for me and it actually harmed my health. The more I ate that way, the more my thyroid function slowed down, the worse my hormones got. I live in this body and I see what goes in my mouth every single day.)And I am trying to get pregnant again. It may be too late, since I'm 4. I know women who had babies at 4. I think I may still have a shot and I really want to try. My hormones being effed up is not something I can afford. How Low Carb Can Be Harmful to Your Hormones. Just the other day, Chris Kresser did an interview with Chris Masterjohn on his podcast. You can go here to listen or read the transcript. In the podcast, Chris Masterjohn said: “I think that if you find that T3 or reverse T3 are out of whack, probably the best way to address that is to try increasing the carbohydrate intake — not necessarily meaning you have to go on a high- carbohydrate diet, but, you know, like, Paul Jaminet had sort of concluded at the end of that series that he still advocates a low- carbohydrate diet, but it’s possible to go too low for some people, and that’s when you might get deficiency in thyroid signaling.”Chris Kresser responded: “And I definitely see this, Chris, in my practice, and this is purely anecdotal, but I often get people who come to me who have been on a low- carb Paleo Diet, not for any particular reason, just because that was their understanding of the Paleo Diet, you know, as a low- carb approach. Eat More Carbs! Then Chris told a really fascinating story from Dr. Price's book, Nutrition and Physical Degeneration. Chris said: He says: “For the Indians of the far North this reinforcement” — he’s talking about reinforcement of nutrition for pregnancy — “was accomplished by supplying special feedings of organs of animals. Folks, we are not living near the Arctic circle (well some of you may be, but I sure as heck am not). We have access to carbohydrates! We don't need to kill moose and harvest their thyroid glands in order to get pregnant. We don't need to take Armour thyroid or maca or sixteen other supplements. We can just eat some waffles! So this isn't just about women's fertility. If you are a man on a low carb or paleo diet and your testosterone is low or you lost your mojo, there's a reason for that. Paul Jaminet Concurs. This brought to my mind a post Paul Jaminet wrote a couple weeks ago on his Perfect Health Diet blog. He said that a higher carb diet is actually better for people who want to have children. Jaminet writes: For most people, I believe a slightly carb- restricted intake of 2. Most people are not currently seeking to have children or engaging in athletic competition. And there is room for difference of opinion about the optimal carb intake. Here's what I have to say to that: Bring on the pizza and nachos! What If You're On the GAPS Diet? I wrote a post about this a few days ago: GAPS Diet Myths. It is absolutely not necessary to be low carb on the GAPS Diet. If you do well on low carb and are on GAPS, then stick with it. But if you are experiencing hormonal problems, you may want to consider increasing your carbs. I'll write a post soon with some suggestions for how to increase your carbs on the GAPS Diet.
